DOT SAP Evaluation

Substance Abuse Professional evaluations for safety-sensitive employees working through the DOT Return-to-Duty process.

Typical use case: For CDL drivers and other DOT-regulated employees following a positive test, refusal, or other prohibited-conduct determination.

Non-DOT Employment Evaluation

Non-DOT Employment Evaluations support employees referred by their employer following a workplace drug or alcohol test issue when the case is not governed by federal DOT regulations.

Typical use case: For employees referred directly by an employer where the position or evaluation is not covered by federal DOT regulations.

Court & Probation Evaluation

Booking, payment & portal

How Booking, Payment, and the Secure Client Portal Work

Clients begin by selecting the appropriate evaluation service and choosing an available appointment. After the appointment is selected, the client proceeds to secure online payment. Once the booking and payment are confirmed, the client receives access instructions for the Sessions Health Client Portal.

Sessions Health is used for secure intake forms, consent forms, document uploads, appointment information, telehealth access, secure messaging, and report delivery where applicable.

Scheduling

Choose an available appointment date and time through the website's scheduling flow.

Secure Payment

Complete full payment in advance by credit or debit card, processed securely by Stripe, before the appointment is confirmed.

Sessions Health Portal

Complete secure forms, upload documents, attend telehealth appointments, and receive follow-up information through the client portal.

Technical note

The production WordPress website will connect the appointment calendar and payment workflow through a custom booking plugin. Sessions Health will remain the secure system for sensitive intake, documents, telehealth, and client communication.

You may securely upload any applicable documents to assist us in preparing for your appointment.

Documents required may vary depending on the service and requesting party.

Government-Issued Photo Identification

To help verify your identity before the evaluation, you will be asked to securely provide clear, high-resolution images of the applicable government-issued photo identification.

  • DOT SAP evaluation

    Commercial Driver's License (CDL) — clear, high-resolution images of both the front and back.

  • Court / court-ordered evaluation

    Passport or State-Issued Photo ID — provide a clear, high-resolution image of the identification document. Where the ID has information on both sides, provide both the front and back.

  • DMV / license reinstatement evaluation

    State-Issued Photo ID or driver's license — provide a clear, high-resolution image of the identification document, including both sides where information appears on the back.

  • Non-DOT employment evaluation

    Passport or State-Issued Photo ID — provide a clear, high-resolution image of the identification document, including both sides where applicable.

  • For your privacy, identification documents and other sensitive records must be submitted through the secure client portal rather than through a standard website form.
  • Government-issued photo identification

  • Court order, referral, or other court paperwork

  • Related legal documents, including prior arrests, charges, or case summaries

  • Motor Vehicle Record (MVR) or DMV report

  • Forms from the referring agency that require the evaluator's signature

  • Probation or pretrial program requirements

  • Certificates or records of completed classes, treatment, education, or other programs

  • Testing or laboratory reports related to the case from the previous 12 months

  • No documents are uploaded on this website

    For privacy and security, applicable records and sensitive supporting documents are submitted through the secure client portal. Please do not upload these documents through a normal public website form.

    Report delivery

    How Your Report Is Delivered

    The secure client portal handles intake, forms, uploads, messaging, and telehealth. Final reports are delivered separately to authorized recipients.

    Intake forms, consent forms, document uploads, secure communication, and telehealth video appointments.

    Secure Report Delivery

    Authorized reports and documentation are securely delivered as PDF files to the appropriate employer, Designated Employer Representative (DER), Third-Party Administrator (TPA), defense attorney, court, or other approved requesting party. For DOT SAP cases, the dates of the initial and follow-up evaluations will be logged in the FMCSA Drug & Alcohol Clearinghouse, as applicable.

    Where appropriate and authorized, DOT SAP documentation may be securely delivered as a PDF to the employer, DER, or designated third-party administrator.

    Court-related reports

    Court evaluation reports may be securely delivered as PDF documents to the authorized defense attorney, court, or other approved requesting party.

    Authorization required

    Reports and records are released only with valid written authorization, or where release is required or permitted by applicable law and regulation. Documentation is never sent to a party without authorization.
